Full Job Description
Under the direction of the DON the home visit RN will manage and oversee a caseload of clients to deliver comfort and excellent care as required.
Responsibilities and Duties

• Provide skilled nursing care to pediatric and adult patients in a home setting Implement, develop, and document the plan of care to ensure quality and continuity of care for each patient
• Provide effective communication to patient/family, team members, and other healthcare professionals
• Implement plan of care and treatment using sound judgment and critical thinking Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process
• Administer oral, IM, Subcut prescribed medications, also treatments, and therapies
• Participate in staff meetings to exchange ideas and information as it relates to the quality initiatives.
