Job Description
We are seeking a Family Medicine Physician to join our innovative care team in West Point, GA. Deliver whole-person care in 30- to 60-minute appointments, supported by a health coach and care team, focusing on high engagement, managed care, and culture transformation to reduce costly service utilization and improve overall health outcomes.
Position Highlights: See 10-15 patients daily in longer, meaningful appointments. Collaborative, team-based approach with health coaching support. Focus on proactive, value-based care
