Job SummaryAtlas is seeking a compassionate and organized Patient Care Coordinator to support residents in our partner communities. This role ensures smooth coordination between patients, families, providers, and facility staff-delivering excellent care and a positive patient experience.
ResponsibilitiesGreet residents with a positive, professional attitude
Enter health info into electronic medical records (EMR)
Schedule appointments and coordinate care efficiently
Communicate updates with residents, providers, and staff
Track patient progress for 90 days post-hospital stay
Refer to Bridge program as needed after 90 days
Assist with compliance and care coordination
Provide statistical reports to the Administrator
Support facility and provider as needed
Follow documentation, billing, and regulatory policies
GED or equivalent
1–2 years of medical or healthcare experience preferred
CNA or MA license preferred (not required)
Valid driver's license + reliable transportation
Proof of current auto insurance
Ability to work with diverse populations
Strong communication and organizational skills
Basic computer and EMR knowledge
Professional appearance and adherence to dress code
Must maintain patient confidentiality at all times
Interpersonal skills
Attention to detail
Proficiency with scheduling and administrative tasks
Familiarity with healthcare compliance standards
