Job Title: Registered Nurse – Case Manager
Location: 195 Riverbend Dr Ste 5, Charlottesville, VA 22911
Coverage Area: Charlottesville to Staunton
Employment Type: Full-time (40 hours/week)
Shift Schedule: Monday to Friday, flexible hours within 8:30 AM – 5:00 PM
Compensation
Primary Responsibilities:
• Conduct comprehensive assessments for high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Reassess care levels for cost-efficiency and appropriateness
• Coordinate non-medical support (e.g., housing, transportation) to ensure treatment compliance
• Manage care transitions with physicians, social workers, discharge planners, and claims teams
• Engage specialty resources to support optimal patient outcomes
• Maintain detailed documentation of clinical, functional, and fiscal results
• Promote health and prevent illness through proactive interventions
• Act swiftly to minimize adverse patient outcomes
