WEEKDAYS | 8:30 a.m. - 5:00 p.m.

JOB SUMMARY

The Community Health Educator is responsible for teaching people about behaviors that promote health and wellness in the Indian Health Board (IHB) community. The Community Health Educator is also responsible for the learning experiences that provide individuals served by IHB the opportunity to make appropriate health decisions. The Community Health Educator will use appropriate strategies, and methods to facilitate health promotion and disease prevention of sexual health. They must have a thorough understanding of the diverse populations served by IHB.

ESSENTIAL FUNCTIONS

  • Plan, organize and deliver the educational components of the Family Planning Special Projects (FPSP) intervention and local intervention activities within the community.
  • Conduct health education with patients and their families in various settings to include time spent in the community such as attending community events and powwows. Approximately 50% or more of educational activities will be conducted in the community setting.
  • Implement and manage health education strategies, interventions and programs.
  • Assess patient needs for health education, including readiness for change.
  • Help develop, identify, implement and improve patient education material and other tools that encourage healthy decisions.
  • Translate medical language and concepts into clear, simple and understandable information for patients to maximize the potential for them to take reasonable action.
  • Develop, monitor and assess health communication interventions and policies.
  • Maintains patient confidentiality and documents (office visits and community) patient education specific to FPSP in the electronic health record.
  • Work collaboratively with IHB staff to coordinate activities associated with health and wellness.
  • Collaborate with practitioners to host/ co-host educational classes for Native American women at high risk for unplanned pregnancy.
  • Participate in Women's Health Day events, including outreach for pap smears and mammograms.
  • Provides excellent customer service to patients, peers, leadership, and community partners.
  • Evaluate the effectiveness of programs and educational materials per grant work plan and by utilizing pre and posttest.
  • Assist with training health program and clinic staff for core competency and health care specific content as needed.
  • Position will work with the Health Programs Supervisor to collect and compile data for grant reporting.
  • Participate in Performance Improvement activities as needed.
  • Perform other duties as assigned by supervisor.

ADDITIONAL RESPONSIBILITES

  • Knowledge of and respect for American Indian cultures and other cultures served by IHB.
  • Proficiency with Microsoft Office applications, especially Excel, Access, and Project
EDUCATION AND EXPERIENCE
  • Bachelor's degree in health education or related field from an accredited institution
  • Valid driver's license with good driving record
  • Experience working with American Indian communities
  • Knowledge of American Indian history and cultural traditions
  • Familiar with local resources preferred
  • Experience with providing training, coaching, or health education
  • Proven ability to lead outcome-based activities.
  • Strong written and verbal communication and interpersonal skills
  • Detail-orientated and self-motivated
  • Evaluate the effectiveness of programs and educational materials
  • Strong organizational and clerical skills, including use of PC, Excel, Word and EMR
  • Ability to show initiative and solve problems, manage one or more projects, be accurate and attentive to deadlines while working independently
  • Excellent networking, leadership, verbal and written communication skills

IHB makes reasonable efforts to limit access to and use of protected health information (PHI) by employees to the minimum necessary performance of assigned duties as outlined in job descriptions. This position is access restricted to PHI needed to provide health education and carry out health care operations in assigned clinical area.

The work requires some physical exertion such as long periods of standing, walking, and recurring bending, crouching, stooping, stretching, reaching, or similar activities. The work may require specific, but common, physical characteristics and abilities such as above average agility and dexterity. An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, blood borne pathogens, physical hazards (e.g. needle pricks, etc.) and chemical hazards
