We are seeking a Registered Nurse for our Outpatient Program of All Inclusive Care for the Elderly (PACE) Program for our team to ensure that we continue to provide all patients with high quality, efficient care. Under the direction of the Clinic Manager, he/she shall work as a member of a strong team to provide nursing care for the comfort and wellbeing of all participants. Triages participants in center and community; administers medications per MAR and facility standing orders; performs initial, semi-annual, and annual assessments; assists MD/NP with ordering of labs, procedures, and consultation visits. Provides wound care and assists with orders of; serves as liaison between providers, participants, staff, and families. Documents care delivery, patient condition, reaction and progress. Utilizes the Nursing Process (Assessment, Planning, Implementation and Evaluation), in the delivery of professional nursing care to the clinic population in accordance with physician practices policies and procedures. Provides patient education, especially in regard to pharmaceuticals and supports the business goals of the practice. Makes adjustments to careplan as indicated to reflect appropriate care and needs being met as well as every 6 months. He/She will also be responsible for being a licensed member of our “On-Call” team providing triage support via phone. Team member will work in conjunction with clinic staff to provide care in clinic.
What qualifications you will need:
Skills, Knowledge, Abilities :
Required Education : BSN degree with an RN license and BLS Certification as well as Frist Aid
Required License : RN license
Required Experience : Minimum of 1 year working with the frail or elderly.
Preferred Experience : Minimum of 3 years of nursing management in a community home based setting preferred.
